HEALTH/DENTAL/VISION INSURANCE

Pitt Plastics recognizes that its’ employees are what make the company successful. So it is only fitting that employees, along with their families, should have access to a top-notch array of life enhancing benefits.

Pitt Plastics continues its commitment by offering eligible employees a broad list of benefits. This provides each individual an opportunity to select specific levels of coverage that will best fit their lifestyle, family needs, and financial responsibilities.

Benefits are available to all full time active employees of Pitt Plastics and their dependents.

MEDICAL INSURANCE:

Pitt Plastics offers two different types of medical plans to their employees, depending on their needs, through Blue Cross Blue Shield of Kansas City, the Buy-Up Plan and the HRA Plan.

The Buy-Up Plan is an 80/20 plan, with a deductible of $500 per person and $1500 for family. On the Buy-Up plan, office visits have a copay of $20.00; preventive care is paid at 100% with no maximum; urgent care visits have a $75.00 copay; emergency room visits have a $150 copay, then deductible and coinsurance is charged; prescription drugs are paid at either $10.00, $20.00, or $50.00, depending on where the drug falls in the formulary.

The HRA plan has a $3,000 individual deductible and a $6,000 for family deductible. Pitt Plastics contributes the first $500 of the deductible on the employee only plan, and contributes the first $1,000 of the deductible on the family plan. Once the Pitt Plastics employer contribution is met, the employee is responsible for the amount up to the deductible. Once the full deductible has been met, claims are paid at 100%. On the HRA Plan, there are no copays and everything is subject to deductible (with the exception of Preventive Care). The prescription drug program is also exempt from the deductible and are paid at either $10.00, $20.00, or $50.00, depending on where the drug falls in the formulary.

DENTAL INSURANCE:

Pitt Plastics also offers to its employees dental insurance provided by Delta Dental of Kansas. The dental plan is an 80/20 plan on Basic, Major, and Orthodontics. Diagnostic and Preventive Care is paid at 100% and provides two (2) cleanings per calendar year. Full mouth x-rays are covered once each every 5 years; diagnostic x-rays are covered once each 12 months; topical fluoride is covered once each 6 months for dependent children under age 19; space maintainers are covered for children under age 19 to replace primary teeth; sealants are covered once every 36 months dependent children under age 14 when applied only to permanent molars. The maximum benefit payment for all covered dental procedures for each Eligible person in any one calendar year is $1000.00.

VISION COVERAGE:

Eye Examinations are paid at $75.00 per year for the employee and $50.00 for dependents on the plan. Lenses are paid at $35.00 per year. This applies to both the Buy-Up Plan and the HRA Plan.

All benefits are subject to the Plan’s terms and conditions, including Deductibles and Coinsurance and Reasonable and Customary Charges, unless otherwise noted or those negotiated fees if offered through a Preferred Provider Organization (PPO) contract for that benefit. Benefit percentages payable by the Plan may change depending upon whether  covered services are obtained from a provider or facility who is a participating member of your Plan’s PPO. 

PARTICIPATING MEMBERS OF THIS PPO MAY CHANGE FROM TIME TO TIME. THEREFORE, IT IS IMPORTANT TO VERIFY THAT THE PROVIDER WHO IS TREATING YOU IS A CURRENT MEMBER

SPECIAL NOTE TO REMEMBER: An employee has 30 days from date of a qualifying event such as a newborn, marriage, divorce, or loss of coverage to get that information to the Benefits Administrator. If information is not received within that 30 day period, the dependents may not be eligible to enroll until the next open enrollment period, which falls in December.

EYE MED

Pitt Plastics has available to its employees an additional vision insurance coverage. The Eye Med plan allows the employee to improve their health through a routine eye exam, while saving the employee money on eye care purchases.    

Through the Eye Med program, an eye exam costs the employee/dependents a $10.00 copay. Lenses through the Eye Med program cost a $25.00 copay; and an allowance of $120.00 is given for frames.

FLEXIBLE SPENDING ACCOUNT (FSA)

Flexible spending accounts provide you with an important tax advantage that can help pay for health care and dependent care expenses on a pre-tax basis. Pitt Plastics provides Medical Reimbursement and Dependent Care FSA’s. Flexible spending accounts are available to all eligible full time employees.  

The current annual maximum amount that an employee can contribute to the Medical Reimbursement account is $2,500. The annual maximum amount that an employee can contribute to the Dependent Care plan is $5,000.

CARE AROUND THE CLOCK

Pitt Plastics offers to its employees on-site physician services. The physician is in the office two days a week. Covered employees and their dependents can access the physician’s office at no charge. There is no office copay; prescriptions filled in the office are free; and lab work performed in the office is free. Wellness checks are performed annually with a full blood work up and a personalized report mailed directly to the employees’ home. 

This service can save the employee hundreds of dollars in health care costs.

COBRA

The Consolidated Omnibus Budget Reconciliation Act (COBRA), a federal law, requires that most employers sponsoring group health plans offer covered employees and their covered spouses and dependent children the opportunity for a temporary extension of health coverage at group rates. This coverage is available in certain instances, called “qualifying events”, where your rights and obligations are under the continuous coverage provisions of the law.

If you are an employee of Pitt Plastics, covered by Pitt Plastics, Inc., Group Health and Dental Plan, you have the right to choose continuation coverage if you lose your group health coverage because of a reduction in your work hours or the termination of your employment (for reasons other than gross misconduct on your part). You will be notified in writing should you become eligible for COBRA coverage.

401(k)

  • Employees are eligible after 60 days of  continuous employment
  • Must be 21 years of age.
  • Entry dates are January 1 and July 1 of each year.
  • Pitt Plastics matches 50% of your contribution up to 5% each time you invest.
  • Pitt Plastics offers two types of deferrals into the 401(k) plan, Pre-Tax Deferrals and Roth Salary Deferrals
  • You are always 100% vested in the contributions you choose to defer. You are vested in Pitt Plastics, Inc., contributions based on years of vesting service. You are 100% vested when employed at Pitt Plastics, Inc., 6 years or more.

Pitt Plastics also offers to its employees the following types of insurance coverage:

  • Basic Life Insurance - company paid policy.  Dependent upon your wage, amounts are 10,000; 20,000, and 50,000. Goes into effect the month you are eligible for benefits, even if you elect not to take out the health insurance coverage.
  • Accidental Death & Dismemberment – available in amounts of $25,000 up to $500,000 for employee; employee & child; employee & family.
  • Voluntary Term Life - available in increments of $10,000 up to $500,000, not to exceed five times your earnings.   At initial enrollment, the guaranteed issue amount is $100,000 without providing evidence of good health.
  • Spouse Supplemental Life – available in $10,000 increments up to a maximum of $250,000, not to exceed 50% of Employee Basic/Supplemental Life Benefit. Spouse guaranteed issue amount at initial enrollment is $20,000.
  • Child Supplemental Life – available for children ages 6 months to 19 years of age in increments of $2,000 up to $10,000 per child. Child guarantee issue amount at initial enrollment is $10,000.
  • Short Term Disability – this policy is provided through Mutual of Omaha that helps provide you with a source of income if you become disabled due to a sickness or off-the-job injury. Monthly benefits range from $500 - $3,000 subject to income requirements.     
  • Long Term Disability – the core plan is provided to you by Pitt Plastics which replaces 40% of your monthly income loss to a maximum of $7,500. You have the option to purchase an additional 20% of income protection. With this buy-up protection, the plan replaces 60% of your monthly income loss to a maximum of $7,500. 
  • Eye Med Vision Plan – this is an optional insurance provided through Eye Med; this insurance supplements the vision plan.
  • Personal Accident Plan – this policy is provided through UNUM and is designed to help cover the expenses associated with an accidental injury. It pays you directly, unless you assign the benefits, regardless of any other insurance you may have.
  • Personal Cancer Protector Plan – this policy is provided through UNUM and pays for various cancer treatments/benefits.
